Please read the following information carefully.
Failure to Provide the right documents will mean that your medical will need to be rescheduled.
Please check your junk mailbox in case the confirmation email has gone there. All of your location-specific information will be on there.
Please bring the following with you to your medical:
- Your medical form – We have most of the medical forms however some the patient has to bring. These include North Somerset, TFL/ PCO and Dudley council (as you need an original form from the council).
- Medical records- *Mandatory for most Taxi Medicals including Wolverhampton even if you don’t have any medical issues – See the list below for what medical records your council requires. Please Note: The council does sometimes change this. YOU CAN DOWNLOAD THE REQUEST FORM HERE
- The glasses or contact lenses you use when driving. If you wear glasses please bring these instead of contact lenses
- Your current eyesight prescription (if you wear glasses to drive)
- Repeat prescription / The medications themselves
If you take insulin / Gliclazide / Glomepramide then you need to bring in your blood glucose monitor. Without the appropriate blood sugar monitoring we will need to reschedule you.
If you haven’t paid online you can pay on the day with cash or card.
If you have had a heart attack or stent in your heart artery you need a treadmill test please whatsapp us for more information.
Here are the additional documents for each council we have done – If yours is not on the list then please check your medical form or whatsapp us:
Wolverhampton and Shropshire Council:
- Driving licence or passport
- Utility bill or bank statement (can be electronic)
- Medical records – Full medical records (printed, electronic or full online access via your GP) NOT GP SUMMARY AND NOT NHS APP – The council will return your forms if we do not have access to your full medical records.
Solihull
- Utility bill or bank statement (can be electronic)
Knowseley
- Driving licence or passport
- Utility bill or bank statement (can be electronic)
- GP summary printed from your GP or electronic copy
Walsall
They must first try their GP if unable to get an appointment, they speak to the council and the council will then recommend us.
- Needs GP summary printed from your GP
Sandwell
- 2 years worth of medical history
- Utility bill or bank statement (can be electronic)
Worcester Council:
- Patient needs to bring their full medical records. This has to be from their GP. The GP receptionist needs to sign Part C of the medical form to say that they have handed over medical records to you.
Dudley council
- The patient needs to bring the form with them from the council as the form has the patient’s photo printed on it
- Utility bill or bank statement (can be electronic)
- Full medical records printed (can take 30 days). Not NHS app and not GP Summary
Stroud, Cherwell and Wychavon Council:
- GP summary or an unlocked NHS app
Tewkesbury Medical
- No special requirements
Lichfield Medical
- Driving licence or passport
- Utility bill or bank statement (can be electronic)
South Gloucester
- Same as D4, just ID and medication
Gloucester City Council
- (Full Medical Records Needed) – May take 30 days to get from your GP. NOT NHS APP and NOT SUMMARY.
- Urine sample – Not all locations have toilet facilities
Ashfield
- Please bring a photocopy of your licence so the doctor can sign it.
Bristol and Swindon Council Taxi Medical
- (Full Medical Records Needed) – May take 30 days to get from your GP. NOT NHS APP and NOT SUMMARY.
Cheltenham, Wiltshire and Warwick Taxi Medical
- No additional requirments
Wyre Forest
- No additional requirements

Cancellations / Rescheduling
Please book another appointment here. The new booking can be cash if you have paid already and we will not take a payment from you on the day. Then email us at hello@simplymedicals.co.uk to let us know you have re-booked and if you had paid or not so admin can apply your fee to the second booking.
Your medical may take place in an office, Porta Cabin, Mobile Clinic, Haulage yard or Medical Centre.
