Failure to Provide the right documents will mean that your medical will need to be rescheduled.
Please check your junk mailbox in case the confirmation email has gone there. All of your location-specific information will be on there.
Please bring the following with you to your medical:
- Your medical form – Haven’t you got one? Don’t Worry! We have spare forms.
- Passport or driving licence
- The glasses or contact lenses you use when driving
- Repeat prescription / The medications themselves if you take medication
- Urine sample – As not all locations have toilet facilities#
If you have booked your medical with a 12-lead ECG we will carry this out at the same time as your medical.
If you require an exercise ECG please contact our trusted cardiologist directly Dr. Hamad, who is based in Walsall or Edgbaston – 0121 396 1216. He can normally get you in within a week.
If you haven’t paid online you can pay on the day with cash or card.

Cancellations / Rescheduling
Please book another appointment here. Then email us at hello@simplymedicals.co.uk to let us know you have re-booked and if you had paid or not. The new booking can be cash if you have paid already.
Your medical may take place in an office, Porta Cabin, Mobile Clinic, Haulage yard or Medical Centre.
