What Happens Next?
If you have NOT uploaded your consent form already please email a copy to : firearms@simplymedicals.co.uk.
You can download a copy here.
If you uploaded a copy on our contact form we will now begin the process of obtaining your medical records.
Once received, we will send your completed consent form to your GP.
We will then need to wait for them to reply.
We will be in contact once your medical records are obtained and the medical proforma is complete. We aim to complete the medical proforma within 3 working days of receiving your medical records.
How can you get the forms done quickly?
Firstly, if you did not upload a copy of your consent form on the booking form please email us a copy to firearms@simplymedicals.co.uk.
Your GP is supposed to send us your medical records within 30 days, it can be sooner if the patient pushes for it. They normally listen to the patient, rather than us just sending them a request on your behalf. There is a legal obligation for them to turn it around in 30 days based on GDPR regulations.
Please Note: We have had instances where GP practices have taken over 30 days, sometimes even over 60 days. So it really does help if the patient push for it.
Another way to speed the process up is for you to drop the original copy of your consent form to the GP practice by hand. This way it does not get left at the bottom of the pile.
