D4 medical is an essential health check for anyone applying for, or renewing, a licence to drive a lorry, bus, or large commercial vehicle. Although the form can look intimidating at first glance, the process is actually very straightforward. What Is a D4 Medical?

A D4 medical is a health assessment required by the DVLA to ensure that professional drivers are medically fit to drive safely. It applies to people applying for or renewing:

  • Group 2 licences (e.g., HGV, LGV, PCV)
  • Bus and coach licences
  • C1 licences (including for ambulance work)
  • Drivers over 45 renewing their licence

It must be completed by a doctor registered with the GMC, or by an optician for the vision section.

The aim isn’t to disqualify drivers — far from it. The DVLA simply wants to ensure that any health issues that might affect driving are properly managed.


What Tests Are Included in a D4 Medical?

The assessment includes several simple checks, none of which are painful or complicated:

1. Vision Test

You’ll be asked to read an eye chart. If you wear glasses or contact lenses, bring them with you. Your optician can fill out this section too.

2. Blood Pressure

High blood pressure is one of the most common reasons a medical needs repeating. Good preparation helps — avoid caffeine and smoking beforehand.

3. Medical History Review

The doctor will ask about:

Be honest — in most cases, conditions are manageable and do not prevent obtaining a licence.

4. Physical Examination

This includes:

  • Urine test (usually for diabetes indicators)
  • Basic heart and lung check
  • General health assessment

  • Your current prescription/glasses
  • Any hospital letters
  • A list of medications
  • Your driving licence number
